For pre-screening purposes, please send the required info on my booking form. please make sure you use an email address that you check regularly. Also, use your real number this is for screening purposes only and I will not contact you via text without your permission.
Once I have had a chance to review your request, l will follow up with you so that we can complete the screening process, and check any remaining appointment details.
